Masking relies on constant self monitoring, suppression of natural responses, and pushing through fatigue or overwhelm. For people with ADHD, this often means compensating with anxiety, perfectionism, overworking, or people pleasing to meet expectations. That effort is cognitively and emotionally expensive. Over years, it increases stress load and reduces access to internal regulation. What we see clinically is not that ADHD itself worsens, but that the compensatory systems burn out. Executive capacity drops, emotional regulation becomes harder, and motivation collapses more quickly. People often describe becoming less resilient, more sensitive to demand, and more depleted than earlier in life. This is commonly misinterpreted as symptoms getting worse rather than coping resources being exhausted. There is also an identity cost. Chronic masking disconnects people from internal cues like interest, fatigue, and need for rest. That makes self support harder and increases shutdown or avoidance when capacity is exceeded. Unmasking safely does not mean losing skills. It means reducing unnecessary effort so symptoms become more manageable again.
