During titration, clinicians are looking for a balance point where benefit is clear and side effects are acceptable. Dose changes are guided by patterns, not single good or bad days. First, they assess benefit. Is there meaningful improvement in focus, follow through, emotional regulation, or daily functioning? If there is some benefit but it feels partial or wears off too quickly, that may support a cautious increase. If benefit is solid and consistent, the dose is usually held. Second, they track side effects. Mild, early effects like appetite change, headache, or sleep shifts can be expected and may settle. If side effects are persistent, worsening, or interfering with mood, sleep, anxiety, or physical health, clinicians may lower the dose or change timing or formulation. Side effects that feel unsafe or distressing usually prompt immediate adjustment. They also look at timing. When symptoms improve and when side effects appear helps determine whether the issue is dose related, duration related, or medication specific. Importantly, subjective experience matters. Feeling more focused but tense, flat, or unlike yourself is clinically relevant. Titration is not about pushing to the highest dose tolerated. It is about finding the lowest dose that provides real life benefit with manageable cost.
