Getting to Know Ms. A. Marchini

What inspired your journey as an NP and your focus on mental health?
I began my career 14 years ago as a nurse practitioner working with adolescents, where mental health and eating disorders quickly became areas of deep interest and focus for me. Those early years shaped my understanding of how neurodevelopmental conditions show up across home, school, and family systems, and they sparked my commitment to providing care that is both evidence-based and genuinely compassionate. As a Nurse Practitioner, I’ve always taken a holistic approach-one that integrates clinical science with an understanding of the person’s lived experience, strengths, and goals. About three years ago, I transitioned into virtual medicine, which allowed me to expand my scope and begin working more extensively with adults. That shift opened my eyes to how many people reach adulthood without ever having their mental health struggles recognized or supported. Since then, I’ve dedicated much of my practice to improving access to high-quality mental health assessment and treatment, combining rigorous clinical standards with a patient-centered, strengths-based philosophy.
What is your philosophy or approach when it comes to mental health care?
I approach mental health care with the understanding that it’s a real neurodevelopmental condition, not a character flaw, and I work hard to help patients release the shame they’ve often carried for years. I take a strengths-based view, recognizing the creativity, intuition, and resilience that so many people with mental health challenges possess, while supporting the areas where symptoms create impairment. My treatment plans are multimodal and evidence-based, blending medication when appropriate with skills-based therapy, coaching, and psychoeducation. I tailor care to each person’s goals, life context, and comorbidities, and I rely on collaborative, shared decision-making rather than a paternalistic model. Ultimately, my focus is on improving real-world functioning and quality of life, grounded in empathy for the lived experience of mental health struggles and an awareness of the systems that shape it.
How do you see telepsychiatry improving access to mental health care?
Telepsychiatry improves access to mental health care by removing geographic and logistical barriers, allowing patients-especially those in rural or underserved areas-to connect with qualified clinicians more quickly and with far shorter wait times. It supports more accurate, real-world assessments by letting clinicians observe patients in their home environment and makes care easier for families who struggle with transportation or scheduling. Virtual visits also strengthen continuity by enabling frequent follow-ups for medication titration and monitoring, reduce stigma by offering a more private setting, and lower indirect costs like travel and time off work. Together, these advantages make mental health assessment and treatment more accessible, consistent, and patient-centered.
What's one thing you hope your patients feel after their first session with you?
I hope patients feel heard and cared for.
