Other medications and supplements may interact, while alcohol, cannabis, nicotine, and caffeine can all alter how stimulants feel or perform. Context and expectations play a role as well. Structured routines, sleep quality, workload, and stress levels influence whether benefits are noticeable. What feels “life-changing” for one person may feel subtle or uncomfortable for another. In some situations, a poor or paradoxical response raises the possibility that ADHD may not be the primary driver of the symptoms. If attention difficulties stem mainly from depression, trauma, substance use, or burnout, stimulant medication may offer limited benefit or worsen certain symptoms.
