Your Ontario doctor does not have to use a hidden official sentence to let you pick brand or generic Ozempic; what the pharmacy can dispense depends on how the prescription is written, on Ontario interchangeability rules, and on whether any interchangeable product is actually listed, while public payment is a separate Limited Use question. If the prescription names a brand and does not allow substitution, a pharmacy may dispense only that product; if substitution is permitted and an interchangeable option exists, the pharmacy may offer the interchangeable product according to applicable pharmacy rules. Those mechanics are not a guarantee that a generic or biosimilar equivalent is on the market for your strength and device, so ask the pharmacist what is currently interchangeable rather than assuming Costco or any chain can switch it. Ontario Drug Benefit coverage, including OHIP+ where it applies, follows Limited Use criteria for semaglutide that are written for type 2 diabetes, not for a brand-versus-generic shopping preference, and people who do not meet LU criteria are not funded on that basis. The January 2024 Executive Officer notice also indicated that EAP would not fund Ozempic for other conditions.
Rules, coverage and referral pathways vary by province and can change; confirm the current position with the relevant institution or provincial program.

This is only my personal experience at an Ontario pharmacy, not a provincial rule. My prescription was written as the brand with no substitution, so they would not give me a cheaper option even though I asked. When my doctor later allowed substitution, the pharmacy could discuss what was actually interchangeable that week. I am not saying every store handles it the same way.
