We consider them when someone has significant anxiety, tic disorders, cardiac concerns, substance misuse history, or if stimulants were not tolerated. Some patients also simply prefer starting with something less activating.
Non stimulants like atomoxetine or guanfacine work differently. They build gradually over weeks and may be gentler, especially for emotional dysregulation or hyperarousal. They are not weaker, just slower and sometimes more subtle.
Treatment is never one size fits all. The best starting point depends on the full clinical picture, not just the diagnosis.
