What is currently known about long-term stimulant use in adults with ADHD?

Long term stimulant use in adults has been studied more than many people realize, and overall the data is reassuring when medications are used as prescribed and monitored. Stimulants have been shown to remain effective over time for many adults, without evidence that they “damage” the brain or inevitably stop working. For most people, benefits persist as long as the medication is appropriate for their needs and life circumstances. Clinicians do regularly reassess long term use, not because it’s unsafe by default, but because needs change. Dose adjustments, medication holidays, or switching medications may be considered based on side effects, cardiovascular risk, sleep, anxiety, or life stage. Re evaluating doesn’t mean something is wrong. It means care is being thoughtful and individualized.
