In general, older adults with ADHD do not experience fundamentally different types of side effects from ADHD medications compared with younger adults. The medications affect the same brain systems at any age. What does change with age is the likelihood of co-existing medical conditions and the body’s overall physiological reserve, which can influence how noticeable or clinically relevant certain side effects become. Cardiovascular factors are one of the commoner considerations. Conditions such as hypertension, arrhythmias, or underlying heart disease become more prevalent with age, so increases in blood pressure or heart rate that might be inconsequential in a younger person may warrant closer monitoring in someone in their late 50s or 60s. This does not mean stimulants or non-stimulants cannot be used, but dosing is often more cautious and physical monitoring more deliberate. Other side effects—such as reduced appetite, sleep disturbance, dry mouth, or mild anxiety—are similar across age groups. However, older adults may already be managing sleep issues, weight changes, or anxiety, which can make these effects feel more impactful. Medication interactions are also more relevant, as people are more likely to be taking antihypertensives, antidepressants, or other long-term treatments. For this reason, clinicians typically start at lower doses, titrate more gradually, and review both physical measures and functional benefit carefully. The focus is not age alone, but how ADHD medication fits within the broader medical picture, with the goal of improving quality of life while maintaining safety.
