Stimulants can heighten physical sensations like a racing heart or restlessness, which some people experience as anxiety, especially if they are sensitive to bodily cues. They also look at what kind of anxiety it is. Medication related anxiety often feels more physical and keyed up. Worry related anxiety tends to be more cognitive, with rumination, fear of mistakes, or anticipatory stress that may have existed before treatment. Sometimes medication unmasks anxiety that was already there but previously hidden under ADHD chaos. Context matters too. If focus improves, you may suddenly notice worries or emotional patterns that were always present but drowned out by distraction. That does not mean the medication caused the anxiety, but it may have made it more visible. Clinicians will often adjust dose, timing, or formulation to see if anxiety settles.
