Once I start one thing I can't change gears. Is that a core symptom?

Very often. Task switching and cognitive flexibility are core executive functions. Many adults describe feeling stuck once they start something, even if they need to pivot. Transitions require mental disengagement plus re engagement. That shift is effortful in ADHD. It is not stubbornness. It is neurological inertia. Building transition rituals, like a five minute reset or written next steps, can reduce that friction. Awareness alone can decrease self blame.
