Part VII. Cancellation & Medical Letter Policies
Because appointment times are reserved specifically for you, last-minute changes and missed visits may be subject to a cancellation or no-show fee. In some cases, part of this fee may be credited toward a rebooked appointment; if you have questions or need help rescheduling, please contact our support team. A fee may apply if:
•
•
| Document Type | Pricing | Estimated Processing Time |
| Diagnostic Letter | $50 | Within 7 business days |
| Accommodation Letter – FasTreat standard template | $50 | Within 7 business days |
| Accommodation Letter – your own template | $50 for the first page, plus $20 for each additional page completed | Within 7 business days, if approved |
| Blank FasTreat Letterhead | $50 | Within 7 business days, if approved |
| Other Custom Template or Form | $50 for the first page, plus $20 for each additional page completed | Within 7 business days, if approved |
| Release of Medical Records – standard | Free | Within 14 business days |
| Release of Medical Records – expedited | $99 | Within 7 business days |
If you use your own accommodation template or another custom form, you will need to upload it when submitting your request. The final fee is based on the number of pages your provider actually completes.
Your provider will first review your request. If it is approved, your card on file will only be charged when the completed document is released to you. For multi-page templates, the final charge will be based on the confirmed number of pages completed.
Log in to the FasTreat Patient Portal, go to Profile, and submit your document request using the self-service option.
Requirements may vary between employers, universities and other organizations. Please confirm their requirements before submitting your request and provide any relevant instructions or templates to FasTreat.
These fees and processing times apply to active FasTreat members only. Processing times are estimates and may vary depending on provider approval, the complexity of the request, and whether further information is required.

Providers and Assessment
For children and adolescents, providers pay close attention to behaviour in multiple settings (home, school, social situations) and may use parent and teacher rating scales in addition to online questionnaires. The visit typically includes time with you and your child together and time focused on your child, so the provider can understand their experience in a developmentally appropriate way.
Treatment and Medication
Treatment plans may include medication, parent and child education, school strategies, and lifestyle or behavioural approaches, and these can be combined or adjusted over time.
Medication decisions depend on the provider’s clinical judgement and the prescribing rules of your province. Some stimulant medications are controlled substances and may only be prescribed by certain types of clinicians. Your provider will advise you if a medication cannot be started or must be coordinated with an in-person practitioner.
