If you move and the old shared-care doctor is licensed in another province, that clinician usually cannot keep writing indefinitely for a patient who now lives and fills in the new province. Physicians generally must be licensed in the jurisdiction where the patient is located. Cross-province prescribing is often not straightforward.

After I moved, the old shared-care doctor in the other province would not keep writing as if I still lived there. I had to enrol in the new public plan and find someone licensed locally. The pharmacy in the new city would not treat the old paper as a simple transfer. I felt like I was starting over even though the diagnosis was not new. Only my move, not a portable shared-care promise for anyone else.
