What’s the best way to track ADHD medication effects so I can give my doctor useful feedback?

I’m worried that when my prescriber asks how I’m doing on medication, I won’t know what to say beyond ‘I think it’s okay’. Are there specific things clinicians want patients to pay attention to during the first few weeks? Would a daily log help, and if so, what should I write down—focus levels, appetite, mood, sleep, or something else? I’d feel better if I knew what information actually helps doctors make adjustments.

You don’t need a complex system to give helpful feedback. What prescribers usually find most useful is pattern-based information rather than perfection. Paying attention to a few consistent areas makes things clearer, such as focus, task initiation, emotional regulation, appetite, sleep, and how long the medication feels effective. Noting when you feel “more like yourself” versus overactivated or flat can be just as important as productivity. Tracking can be as simple as a short daily note or weekly summary. For example, “Focus improved from 9–2, crash around 4, appetite low until evening.” Concrete examples help more than general statements. It’s also helpful to note context, like stress or sleep deprivation, since these can affect how medication feels. The goal isn’t to prove anything, it’s to give your clinician enough information to fine-tune treatment.
