They can point at the same ADHD question and still be different documents produced for different systems. A psychiatric or medical assessment is done by a physician or other medical prescriber. It includes history, differential diagnosis, medical review, and treatment decisions, including whether medication will be used. FasTreat also offers private online ADHD assessment and ongoing care, depending on your needs and clinical assessment. I paid for testing thinking it would double as the medical diagnosis and the school paperwork. I ended up with a psychology report that my workplace liked and a separate medical visit I had not budgeted time for. I was grouchy about the duplication. Nobody had promised it was one document except me, in my head. I still mix the two folders up. Personal expense and confusion, not a template for anyone else's assessment path.

School and the psychiatrist wanted different stacks of paper, which I found almost funny once I stopped being angry. I had waved the psychiatry letter at the school and got a polite request for psychoeducational testing anyway. I felt like I was collecting stamps. I did not take that as the psychiatrist being wrong. It was two offices speaking two dialects. Just my year of PDFs. I would not tell another parent their school must accept a medical letter.
