Population studies suggest women with PCOS have higher rates of ADHD compared to the general population, and women with ADHD are more likely to meet criteria for PCOS. Meta analyses estimate ADHD prevalence in women with PCOS to be roughly 1.5 to 2 times higher than expected. This pattern has been replicated across different countries and study designs. Hormones appear to be a key link. PCOS involves androgen excess, insulin resistance, and altered estrogen progesterone balance. Dopamine signaling, which is central in ADHD, is sensitive to sex hormones. Estrogen in particular supports dopamine availability and executive functioning. When estrogen fluctuates or is relatively low, ADHD symptoms often worsen. This helps explain why many women notice changes across the menstrual cycle, postpartum, or with hormonal conditions like PCOS. Insulin resistance and chronic inflammation, common in PCOS, may also affect brain energy regulation and attention. Clinically, many women report ADHD symptoms becoming more noticeable after puberty, worsening with hormonal instability, or improving when PCOS is better managed. Importantly, PCOS does not cause ADHD, and ADHD does not cause PCOS. They appear to share overlapping biological vulnerabilities. Recognizing this overlap helps clinicians avoid dismissing symptoms as purely psychological and encourages integrated care that considers hormones, metabolism, and neurodevelopment together.
