I have intrusive thoughts and checking behaviours but also a lot of general worrying. In practice, how do you decide whether this is best understood as OCD, generalised anxiety, or both?

We look at the structure of the thoughts and behaviors. OCD involves intrusive, unwanted thoughts that cause distress and lead to compulsive behaviors or mental rituals to reduce anxiety. GAD tends to involve more diffuse, everyday worries about multiple areas of life without specific compulsions. The presence of repetitive checking, reassurance seeking, or rigid mental rituals suggests OCD. Careful history taking matters. Treatment differs, so accurate differentiation is important.
