Is medical weight management actually covered by provincial health insurance like OHIP or MSP at all, or is it all out of pocket?

The assessment/consultation itself with a covered provider (family doctor, and in some provinces NPs) is generally covered the same as any other medical visit under provincial health insurance. The medication itself, however, is typically not covered by provincial public health insurance for weight management specifically, that usually falls to private insurance or out-of-pocket payment, unlike some diabetes medications which may have different public coverage pathways.
