Yes. Long term masking can deeply affect identity development, and many clinicians see this pattern clearly in adolescents and adults with ADHD. When you spend years adapting yourself to meet external expectations, your focus shifts from understanding who you are to managing how you are perceived. You learn to scan the room, anticipate reactions, suppress needs, and perform competence. Over time, this can blur the line between your authentic self and the version that feels safest to present. Clinically, we often see people who are highly capable but unsure of their preferences, limits, or values. Decisions feel hard because so much energy has gone into fitting in rather than checking in. Many describe a sense of emptiness or confusion once the mask slips, especially after diagnosis, burnout, or a life transition. Masking can also delay self trust. When your internal experience has been repeatedly dismissed or hidden, you may stop using it as a reliable guide. Instead, you rely on external feedback to tell you if you are doing life “right,” which can fuel anxiety and people pleasing. Importantly, this is not a failure of identity. It is an adaptive response to growing up in environments that did not recognize or support how your brain works. The good news is that identity can be rebuilt. With understanding and safety, people often begin reconnecting with curiosity, boundaries, and preferences they had to set aside. Unmasking is not about becoming someone new. It is about slowly returning to yourself, with compassion for the parts of you that learned to survive by blending in.
