Clinicians hear this description a lot, especially from high functioning adults with ADHD. On the outside, things may look fine because you’re meeting expectations, showing up, and getting tasks done. Internally though, it can feel like constant strain, over effort, and barely keeping your head above water. That gap matters. In assessments, we pay attention not just to outcomes, but to the cost of functioning.
If daily life requires excessive energy, anxiety, perfectionism, or self pressure to stay afloat, that signals compensatory coping rather than true regulation. Many adults don’t seek help until burnout hits because they’ve been surviving, not thriving. Looking fine doesn’t mean things are fine, and clinicians who work with adult ADHD are trained to listen for that internal experience.
