Already have a diagnosis letter but want a provider to actually review my current medication plan — which service covers that?

Have documentation from years ago but haven't had an actual clinical review of my medication in a long time, just been on autopilot getting refills without anyone really checking if the plan still makes sense. Not looking for a full new assessment, just want an actual provider to look at what I'm currently doing and tell me if it's still right.
