When ADHD co-occurs with conditions like anxiety or depression, treatment usually becomes more sequenced and individualized, rather than “one-size-fits-all.” A key principle is treating the symptoms that are causing the greatest functional impairment first. For some people, ADHD symptoms (inattention, disorganization, impulsivity) are the primary driver of distress and may be contributing secondarily to low mood or anxiety. In those cases, treating ADHD effectively can lead to downstream improvement in anxiety or depressive symptoms. In other individuals, however, anxiety or depression is more severe or destabilizing at presentation. When anxiety is high—particularly if there is prominent hyperarousal, panic, or trauma-related symptoms—initiating stimulant medication too early can sometimes worsen anxiety, irritability, or sleep. Similarly, untreated major depression can limit the benefits of ADHD treatment, as low energy, poor motivation, and negative cognitions may persist despite adequate stimulant dosing. As a result, clinicians may first focus on stabilizing mood or anxiety, using psychotherapy (e.g., CBT, trauma-focused therapy) and/or medications such as SSRIs or SNRIs, before introducing ADHD pharmacotherapy. Throughout treatment, careful monitoring is essential, as symptom patterns can shift over time. In many cases, combined treatment—addressing ADHD and comorbid conditions concurrently once stability is achieved—leads to the best functional outcomes.
