Yes, ADHD can absolutely exist without obvious hyperactivity. This is a very common point of confusion, especially because early descriptions of ADHD focused heavily on physical restlessness. In reality, the DSM-5 recognizes three different presentations of ADHD, and not all involve visible hyperactivity. The first is ADHD, Predominantly Inattentive Presentation. People with this type mainly struggle with attention regulation rather than excess movement. Common features include distractibility, forgetfulness, difficulty organizing tasks, procrastination, losing things, and feeling mentally scattered or overwhelmed. Hyperactivity may be minimal or absent, which is why this presentation is often overlooked, especially in adults. The second is ADHD, Predominantly Hyperactive-Impulsive Presentation. This is the least common in adults. It involves restlessness, impulsive decision-making, difficulty waiting, interrupting others, and feeling driven to be constantly “on the go.” This presentation is more often identified in childhood. The third is ADHD, Combined Presentation, where both inattentive and hyperactive-impulsive symptoms are present to a clinically significant degree. Many adults fall into this category, though the hyperactivity often becomes more internal over time, experienced as racing thoughts, inner restlessness, or difficulty relaxing rather than constant physical movement. Importantly, ADHD is diagnosed based on patterns of symptoms and functional impairment, not on how hyperactive someone looks. Feeling unfocused, mentally scattered, and chronically overwhelmed can very much be ADHD.
