Clinically, ADHD impairment is highly context dependent. The same person may function well in one environment and struggle significantly in another. This variability is a defining feature of ADHD, not evidence that symptoms are exaggerated. ADHD brains rely more on external structure, interest, urgency, and feedback. When those are present, regulation improves. When environments are abstract, self directed, repetitive, or poorly structured, symptoms intensify. That tells clinicians the issue is not raw capability, but the fit between demands and supports. This is why accommodations and environmental changes can be as effective as medication. Impairment emerges when context exceeds regulation capacity. ADHD is not an all or nothing limitation. It is a condition that becomes disabling when supports are missing and often recedes when they are restored.
