Low motivation is a common concern, but on its own it is not diagnostic of any single condition. Clinicians distinguish ADHD-related motivation difficulties from depression by looking at the pattern, context, and associated symptoms, rather than assuming one explanation based on motivation alone. In ADHD, motivation problems are usually task-specific and interest-dependent. People may feel mentally capable but unable to initiate or sustain effort for tasks that are boring, repetitive, or lack urgency, while being able to focus well on activities that are stimulating or meaningful. Mood is often relatively stable, and enjoyment is still present once the person is engaged. Motivation may improve with structure, deadlines, novelty, or external accountability. In depression, low motivation is more global and pervasive. It is typically accompanied by persistent low mood, loss of pleasure, fatigue, negative self-appraisal, and reduced emotional reactivity. Motivation does not reliably return with interest or urgency, and impairment tends to affect most areas of life consistently. Clinicians also consider timing and course. ADHD-related difficulties usually have a longstanding history, often dating back to childhood, whereas depressive episodes represent a change from a person’s usual level of functioning. Importantly, low motivation can also arise from stress, burnout, sleep deprivation, medical illness, or environmental overload. Diagnosis is therefore never made on the basis of motivation alone. A comprehensive assessment weighs multiple symptoms, functional impact, duration, and alternative explanations before determining the most appropriate formulation.
