This is one of the most defining and misunderstood features of ADHD. ADHD does not primarily impair ability. It impairs regulation. The skills, intelligence, creativity, and problem solving are there, but access to them is inconsistent because the brain has difficulty reliably initiating, sustaining, and sequencing effort over time. Capability shows up in moments of interest, urgency, novelty, or external pressure. In those states, dopamine and norepinephrine availability increases, and performance can look excellent or even exceptional. Consistency, however, requires stable internal regulation rather than spikes. It depends on time perception, task initiation, working memory, emotional regulation, and energy modulation. Those systems are variable in ADHD. This creates the painful pattern of knowing you can do something, having done it well before, yet being unable to reproduce it on demand. From the outside it looks like willpower. Clinically, it is state dependent access to skills. Consistency also requires predictability of energy. ADHD nervous systems fluctuate more, which makes habits harder to form and routines harder to maintain even when motivation is present. This is why treatment focuses less on teaching skills and more on reducing friction, adding external structure, and aligning demands with brain states. You are not inconsistent because you lack discipline. You are inconsistent because your brain does not deliver effort evenly, even when capability is intact.
