ADHD involves differences in how dopamine and norepinephrine are regulated, especially in networks responsible for attention, motivation, and executive control. These systems are highly state dependent. They respond to sleep quality, stress hormones, emotional load, interest, novelty, physical health, and even light exposure. Small changes in any of these inputs can shift how available your cognitive resources are that day. Neural signaling in ADHD is also less tonic and more phasic. Instead of a steady baseline of activation, the brain relies on spikes triggered by urgency or interest. On days when those drivers are naturally present, symptoms feel quieter. On days when they are absent or when baseline stress is higher, access to focus and initiation drops sharply. Prefrontal cortex function is especially sensitive to overload. Mild stress can help performance, but too much stress, poor sleep, or cumulative fatigue can temporarily suppress executive functioning. When that happens, the brain shifts toward reactive rather than planned behavior. Hormones, inflammation, illness, and burnout further influence this variability. Nothing has to be “wrong” for a bad day to happen. Clinically, fluctuating symptoms are not a sign of inconsistency or exaggeration. They reflect a nervous system whose regulation depends heavily on internal and external conditions. Treatment focuses on smoothing those swings by supporting sleep, reducing load, adding structure, and removing moral judgment from variable performance.
