Yes, difficulty with decision making is a core part of ADHD, and it goes far beyond being indecisive. Decision making relies on several executive functions at once. You have to hold options in mind, compare them, predict outcomes, tolerate uncertainty, and then initiate action. In ADHD, working memory is more limited and regulation is less stable, so those steps feel heavier and more chaotic, even for small choices. There is also a dopamine component. Many decisions do not offer immediate reward, which makes the brain less motivated to engage. When there is no clear best option, the brain can stall. Add fear of making the wrong choice, often shaped by past criticism or mistakes, and avoidance becomes protective. People with ADHD also tend to see too many possibilities at once. Instead of narrowing options intuitively, the brain keeps scanning, which increases cognitive load and emotional stress. What looks like overthinking is often an overloaded regulation system. Clinically, this is not a lack of intelligence or confidence. It is a difficulty with cognitive filtering and initiation. Reducing options, externalizing decisions, and allowing good enough choices are common strategies. Decision fatigue is real in ADHD, and struggling with it is part of the condition, not a personal flaw.
