I can spend hours absorbed in certain tasks, but struggle to focus on things that are important or urgent. How do clinicians explain this uneven focus pattern in ADHD?

This uneven focus pattern is one of the most defining features of ADHD, and clinicians explain it as a regulation issue, not a values or motivation problem. ADHD brains do not prioritize attention based on importance. They prioritize based on stimulation. Tasks that are interesting, novel, challenging, or emotionally engaging trigger dopamine release, which allows sustained focus and even hyperfocus. Tasks that are important but boring, repetitive, or abstract do not trigger that same chemical support, so attention drops, even when the stakes are high. Urgency alone is often not enough unless it creates emotional pressure. That is why people may focus only once a deadline is imminent. The brain needs a certain activation threshold to engage, and importance does not reliably meet it. Clinically, this explains why people with ADHD care deeply about things they struggle to do. The difficulty is not valuing the task. It is accessing focus on demand. This pattern is frustrating because it looks like inconsistency from the outside and feels like self sabotage from the inside. In reality, it reflects interest based attention. Treatment focuses on changing conditions rather than character. Adding stimulation, breaking tasks into activating steps, or creating external urgency helps align focus with what matters. Your ability to focus is intact. It is just unevenly accessible.
