In conversations, I drift, miss details, and then ask people to repeat themselves. It’s hurtful to others, but it doesn’t feel intentional. How do clinicians connect attention symptoms to ‘listening’ problems, and what can I do about it?

Many adults with ADHD worry they’re rude, careless, or not trying hard enough in conversations. Clinicians look at patterns. If you want to listen, care about what’s being said, but still drift, miss details, or lose track mid conversation, that points to attention regulation rather than character. Listening requires sustained attention, working memory, and filtering distractions. In ADHD, those systems are inconsistent. The mind may wander even when interest is there. This is very different from not caring. When attention improves with structure, medication, or reduced distractions, clinicians view this as further evidence of ADHD rather than a personality flaw.
